Re-irradiation of Various Locally Recurrent Cancers Combined with Hyperthermia

*AMC protocol: re-irradiation with 8 fractions of 4Gy, (2 per week) + 1 hyperthermia /week
BVI protocol: re-irradiation with 12 fractions of 3Gy, (4 per week) + 2 hyperthermia / week

Irresectable, loco-regional recurrent breast cancers in previously irradiated areas. Hyperthermia combined with re-irradiation (reRT) (hypofractionated, low dose), two protocols are available internationally, both reporting similar results:

  • AMC protocol: re-irradiation with 8 fractions of 4Gy, (2 per week) + 1 hyperthermia /week

  • BVI protocol: re-irradiation with 12 fractions of 3Gy, (4 per week) + 2 hyperthermia / week

AMC protocol is the standard treatment in the Netherlands. AMC protocol shows a higher rate of toxicity than the BVI protocol.[3]

The BVI protocol is therefore recommended.

Outcomes

  • Local response rate >60% in the absence of other options
